For nearly 50 years, we’ve been empowering older adults to live as independently as possible. With support from our team of experienced, caring, licensed Social Workers, more than 98% of clients utilizing Senior Outreach Services remain independent.
These services are available to anyone age 55+ living in suburban Hennepin County.
Identify Strengths and Needs. It starts with a conversation to identify strengths and needs, and assess areas of daily life where support might be helpful.
Connect to Personalized Resources. Using the assessment, we help each client maximize their independence by connecting them with a personalized combination of trusted, affordable resources to address their unique needs, which may include:
Provide Ongoing Support to Navigate Changes. Clients can continue to contact us for help as their circumstances change. This might include a change in income, loss of a spouse or partner, diagnosis of a new or chronic disease, or a desire to prepare for the future.
