Many of the more than 500,000 unpaid family caregivers* in Minnesota may not view themselves as caregivers, especially those who have gradually taken on the role as a loved one’s needs have changed.
Even if your loved one is receiving personal or medical care from a paid caregiver, caregiving also includes grocery shopping, bill paying, transportation, housekeeping, yard work, pet care and more.
By empowering caregivers with knowledge and tools to assess and prioritize their own wellbeing, they are better able to care for their loved one and manage changing circumstances.
Services are provided by our experienced team of professional licensed Social Workers and open to those caring for an older adult or someone with a cognitive diagnosis; the caregiver or care receiver must reside in our service area which includes Carver, Hennepin, Scott, Sherburne, and Wright Counties.
Caregiver Coaching equips caregivers with knowledge, skills, and tools to perform their caregiver role while achieving a balanced lifestyle.
Caregiver Consultation addresses caregiver concerns about the care receiver:
Coaching and Consultation can take place in person or by Zoom, telephone or email.
Family Meeting Facilitation helps families understand their current situation and plan for the future with realistic expectations, positive communication, and fewer misunderstandings.
Support Groups are designed to help caregivers care for themselves through education and empowerment. They offer a safe place for caregivers to learn from one another by sharing experiences, challenges, coping strategies, and humor. View our calendar of support groups.
Fees vary by type of service, but no one is denied service because of inability to pay.
Funding is provided, in part, by Hennepin County, municipalities, Title III grants from Trellis & CMCOA, and a Live Well at Home Grant from the Minnesota Department of Human Services. Services are also supported by a combination of funds from individual donors, the Older Americans Act, foundation grants, and client cost-share. Some services may also be covered by Elderly Waiver or the MSHO program.
